Two convenient ways to make an appointment:

·       Telephone

In order to expedite the intake process, you may wish to download and complete these forms in advance.

Online: This is the quickest and easiest way to schedule an appointment. Click here and it will link you to the online scheduling site to make an appointment. You may also use this site to monitor cancellations to get an appointment sooner or check when your appointment time/day is. Should you need to cancel or reschedule your appointment, you may do so 24 hrs/ 7 days a week.

Telephone: You may call 623-776-9921 to schedule an appointment. Please note: I am likely to be in session when you call. Leave a message and either my scheduler or I will personally return your call as quickly as possible. If your call is outside of normal office hours, please leave a message and we will return your call the next business day.
